Family Assessment and Planning Team (FAPT)
About the Family Assessment and Planning Team (FAPT)
The Family Assessment and Planning Team (FAPT) develops service plans for eligible youth and families and reviews cases funded by the Children’s Services Act, formerly the Comprehensive Services Act to determine progress, prepare transition plans, and adjust services as needed. The FAPT studies and discusses information from case managers, parents and/or guardians, psychological reports, school records, and other pertinent data sources. The youth’s family is strongly encouraged to attend and participate in the FAPT meeting. Members are appointed by the Community Policy and Management Team. The FAPT includes a representative from the following agencies:
- Orange County Department of Social Services;
- Orange County Public Schools;
- Rappahannock-Rapidan Community Services Board; and
- 16th District Court Services Unit.
In addition, the FAPT membership includes a:
- Parent Representative; and
- Private Provider Representatives.
The Orange County FAPT believes that all children and communities deserve to be safe. We believe in family, child, and youth-driven practices. We believe that children do best when raised in families. We believe that all children and youth need and deserve a permanent family. We believe in partnering with others to support child and family success in a system that is family focused, child-centered, and community-based.
